Print Intake Assessment
As a New Client, we ask that you print and complete the Intake Assessment paperwork before visiting our facility.
Once you have printed the information and filled out the form you can:
- Bring to our main office, 104 Spink Street, Wooster, Ohio or our Millersburg Location, 34-C South Clay Street, Millersburg, Ohio

Welcome Packet
New Client Welcome Packet. We are a dynamic, integrated health system operating multiple facilities.
Release of Information
If you have a referral source or any other party with whom you would like OneEighty to communicate, please print, complete, and sign the Release of Information form and include it with your intake packet.
Fiscal Policy and Procedure for Service Reimbursement*
A completed, submittable form is required for all fiscal transactions and reimbursements.
